Dependent Enrollment Form
If you are already enrolled in a Self and Family plan,
use the link above to add your newly eligible child(ren) to your GEHA coverage.

As a result of the Affordable Care Act, children will be eligible for health coverage under their parent's Self and Family enrollment up to age 26. For the FEHB program, this provision is effective on January 1, 2011.

What this means for GEHA members
The specifics of what you need to do to add your dependents depends on your current enrollment.

If you are already enrolled in a Self and Family plan, you will need to click on the link at the top of this page to fill out a dependent enrollment form and submit it to GEHA. You can also call Customer Service at (800) 821-6136 to request a dependent enrollment form.

If you currently have a Self Only enrollment and want your child covered under the FEHB program, you must change to a Self and Family enrollment through your agency or retirement system. You can do this during Open Season or if you have a Qualifying Life Event (QLE).
