If you are a Health Savings Advantage^SM high-deductible health plan member, the Avesis Advantage Vision Program offers you and your covered family members professional vision care from qualified providers participating with Avesis. Through this program, you and your family members are entitled to routine eye examinations for a $10 copayment per exam. If eyeglasses are prescribed for you by your vision care provider, you may receive eyeglass lenses and a frame, within the plan allowance, for a $10 copayment. Should you want contact lenses in lieu of eyeglasses, you will receive an allowance of up to $110 during the benefit year. The participating providers will extend discounts to you for purchases of cosmetic extras such as tints, special coatings, lens type, etc.

Avesis is also pleased to offer you a LASIK vision option through a network of participating providers. Discounts of up to 25 percent are available in selected areas for the procedure.

Should you choose to use a provider not participating with Avesis, services will be reimbursed according to the Avesis Direct Reimbursement Schedule of Benefits after submission of your paid receipt.
