Beginning January 1, 2008, if you are a GEHA Standard Option or High Option member or a GEHA
dental plan member, you get vision coverage for no additional premium.
Through CONNECTION's Avesis Vision Plan, you and your covered family members each pay
only $25 for an annual eye exam when you use a qualified Avesis participating
provider. Or, if you use a provider that does not participate, Avesis will
reimburse you up to $25 for an annual routine eye exam.

CONNECTION's Avesis Vision Plan also offers you discounts off the retail price
of lenses, frames and specialty items such as tints, lightweight plastics and
scratch-resistant coatings, contact lenses and surgical procedures (including
