Through CONNECTION's Avesis Vision Plan, you and your covered family members each pay
only $5 for an annual eye exam when you use a qualified Avesis participating
provider. Or, if you use a provider that does not participate, Avesis will
reimburse you up to $45 for an annual routine eye exam.
