You have finished the first step in completing the application to enroll in Connection Dental Plus. Now it's time to set up your payment options.
Bank draft payment
- If you selected Monthly bank draft from checking or savings, you are required to complete the Bank Draft Authorization Form. Once received, we will contact your bank to set up automatic draft for premium payment.
Billing payment
- If you selected to receive billing from GEHA Connection Dental Plus Premium, a Welcome Letter and Invoice will be mailed to you.
Confirmation
Thank you for enrolling into Connection Dental Plus coverage.
This is your confirmation of receipt of your application. If you meet the enrollment requirements, your Connection Dental Plus coverage will begin the 1st of the month following receipt of your completed application. Additionally, the benefit will not be active until your premium payment is received.
