You have finished the first step in completing the application to enroll in Connection Dental Plus. Now it's time to set up your payment options.

Bank draft payment
  • If you selected Monthly or quarterly bank draft from checking or savings, you are required to complete the Bank Draft Authorization Form. Once received, we will contact your bank to set up automatic draft for premium payment.
Billing payment
  • If you selected Quarterly billing from Connection Dental Plus as your payment option, we will contact you for payment via email or mailed billing statement. However, your coverage will not begin until the first of the month following receipt of your premium payment.
