Notices of Privacy Practices (NPP) — An NPP explains your rights under HIPAA and how you can get access to your protected health information (PHI). It also describes how we may use and disclose your PHI.

Authorization to Disclose Protected Health Information — You can use this form to give GEHA permission to discuss or disclose your protected health information with a specific person other than yourself.

Access to Protected Health Information — You can use this form to request a copy of your protected health information.
