A yearly personal health assessment is a great way to monitor your health. It’s an online survey that helps identify lifestyle habits that can lead to potential illness. It takes only a few minutes to fill out and is completely confidential.
You’ll receive an instant personal wellness report that includes your “good habits” score and health strengths. It will also identify possible health risks so you know where to make lifestyle changes.
GEHA offers a health risk assessment as part of our wellness rewards programs for members. After completing the assessment, medical plan members who are eligible for the Health Rewards or Wellness Pays program will earn $75 on a reloadable debit card. Rewards can be used for 213(d) qualified medical expenses – such as eyeglasses or contacts, orthodontics, doctor visits and dental care.
HDHP members with a Health Savings Account can use their rewards on qualified dental and vision care expenses until they meet their deductible.
