When you take your annual health assessment as a GEHA medical member, you earn a $75 wellness account credit.*

A personal health assessment is an online survey that helps identify lifestyle habits that can lead to potential illness. It is a way you can learn what you’re doing right and what you need to work on to be healthier.

The questions are related to lifestyle practices and health history factors that have the highest impact on your health, as well as biometric measures to determine health status. Lifestyle questions focus on exercise, nutrition, smoking, alcohol use, stress and well-being. Biometric measures are things like height, weight, blood pressure and cholesterol.

GEHA offers a health risk assessment as part of our rewards program for members. It is a good idea to take the assessment every year to keep tabs on your health. It is easy to take and confidential. After completing it, you will receive an instant personal wellness report that includes your “good habits” score and health strengths. It will also identify possible health risks so you know where to make any lifestyle changes.

After completing the assessment, all medical plan members will receive a $75 prepaid Visa card, which can be used for qualified medical expenses or health-related items. Standard Option, High Option, and HDHP members with a Health Reimbursement Account can spend available rewards on all 213(d) qualified expenses — such as eyeglasses or contacts, orthodontics, doctor visits and dental care — using the Health Rewards card.

Elevate and Elevate Plus members can take a health survey to earn a $75 reward through Wellness Pays.

HDHP members with a Health Savings Account can use their rewards on qualified dental and vision care expenses until they meet their deductible.
